This submission focuses chiefly on the role and processes used by Primary Health Networks (PHNs) in Victoria in the establishment and management of the Care Finder program in Aged Care. We present this as a case study that we believe throws light on strengths and weaknesses of the business model used by PHNs in the commissioning of area-based consumer support programs.
While we have expressed some of these issues in responding to previous reviews in the aged care area, we welcome the opportunity to feed into this more focused review of the working models of PHNs.
The submission commences by making some comments on the roles played by PHNs in supporting aged care and primary health delivery for older Victorians broadly. We appreciate this being a focus for PHNs across Victoria and would like to see it increasingly informed by effective engagement with older consumers and collaboration with peak bodies representing the interests of older people.
We hope that our comments contribute towards future thinking relating to the stated interests of the Review regarding program objectives and activities, regional planning, communication and engagement, and program funding arrangements.
